Vision Summary of Benefits

An employee's initial enrollment for vision coverage with EyeMed takes place during new hire orientation, and coverage normally begins the first day of the month following the date of hire. View the Summary of Benefits (PDF).


The City pays 100% of the cost for employee-only coverage, and employees may cover eligible family members at their own expense.

Making Changes

Changes may be made within 31 days of a qualifying event. Qualifying event examples include marriage, divorce, birth, family member loss of coverage, and death.

What happens if I leave?

If your last day worked is on or before the 15th, your coverage ends at midnight on the 15th. If your last day worked is the 16th or later, your coverage ends at midnight on the last day of the month.

Under the provisions of the Consolidated Omnibus Reconciliation Act of 1985, or COBRA, you and your covered family members may choose to continue vision coverage under the City's vision plan after your coverage is scheduled to end. You have 60 days from the date of your election notice to elect continued coverage. If you do not respond within 60 days, your rights to continue coverage end.
